REQUIREMENTS FOR NEW STUDENT ADMISSION

  • Letter of Intent to Apply for Admission addressed to the School Director
  • PSA birth certificate: original
  • DepEd LRN (Learner Reference Number) for Kindergarten to Grade 11
  • Previous Level’s Academic Report Card with Gov’t Recognition: No failing or incomplete grade
  • Certificate of Good Moral Character OR Letter of Recommendation (for Grades 1-11)
  • Printed Enrollment Form
  • Medical Certificate: For students with physical/health or other conditions that the school needs to know
  • Others:

Foreign Students:

  • Permit to study/student visa/certification from the Bureau of Immigration
  • Original school records of all previous grade levels

Grade 1:

  • Original PSA Birth Certificate
  • SF9/F138/Card (Previous Level)
  • Good moral (for transferees)
  • ECCD Checklist (for Grade 1 only)
  • Signed Student Honor Code
  • Signed Parent Agreement with The Quantum Academy
  • Signed Student Agreement with The Quantum Academy
  • Completely filled out Enrollment form
  • SF10/F137 from previous school (for Grades 2 to 6)
  • Other documents needed

Kindergarten:

  • Original PSA Birth Certificate
  • Completely filled out enrollment form
  • Signed Parent Agreement with The Quantum Academy

Nursery:

  • At least 4 years old by October of the incoming school year
  • Immunization record
